4 And I saw thrones, and they sat upon them, and judgment was given unto them: and I saw the souls of them that were BEHEADED for the witness of Jesus, and for the word of God, and which HAD NOT worshiped the beast, neither his image, neither had received his mark upon their foreheads, or in their hands; and THEY lived and reigned with Christ a thousand years. ...WOW! Jesus said earlier
"...the words that I have spoken, they will be the judge in the last day"
Where do we find the words that Jesus has spoken?
Primarily, in the Gospels( better, see the red letter edition of Scripture)
So , unbelievers will be judged on their unbelief,the sin unto death
It follows that if men in the lake of fire are tormented in the presence of the Lamb forever, they are FOREVER in HIS PRESENCE, not separated from him. So death can not be separation eternal consious torment in the presence of the Lamb, it can only be consious torment until total destruction in the presence of the Lamb, followed by separation (being dead)
